Pakistan faces a diabetes epidemic of staggering proportions. According to the International Diabetes Federation (IDF), approximately 33 million adults in Pakistan — roughly 17% of the adult population — are living with diabetes. This makes Pakistan the third-largest diabetic population in the world, behind only India and China. Even more concerning, an estimated 10 million additional Pakistanis have pre-diabetic conditions and do not yet know it.
The economic burden is equally staggering. Direct and indirect costs of diabetes in Pakistan are estimated at $5.2 billion annually — equivalent to 2% of GDP. For individual families, the cost of insulin, medications, and complications management can consume a significant portion of household income, particularly for lower-income families.

The good news is that Type 2 diabetes is largely preventable. The landmark Diabetes Prevention Program (DPP) study demonstrated that lifestyle interventions — modest weight loss, regular physical activity, and dietary improvements — reduce diabetes risk by 58%. Here are evidence-based strategies:
1. Achieve and maintain a healthy weight. Losing just 5-7% of body weight (for a 90 kg person, that is 4.5-6.3 kg) significantly reduces diabetes risk. Focus on sustainable changes rather than crash diets.
2. Move more, sit less. Aim for at least 150 minutes of moderate-intensity physical activity per week — that is 30 minutes, five days a week. Walking, cycling, swimming, or even brisk household chores count. The key is consistency, not intensity.
3. Choose whole grains over refined carbs. Replace white rice and white bread with brown rice, whole wheat roti, oats, and barley. These foods have a lower glycemic index, meaning they raise blood sugar more gradually.
4. Increase fiber intake. Pakistani cuisine naturally includes many high-fiber foods — lentils (daal), chickpeas (chana), vegetables, and fruits. Aim for at least 25-30 grams of fiber daily.
5. Reduce sugar and sweetened beverages. Pakistan has one of the highest per-capita consumption rates of sugary drinks in South Asia. Replacing soft drinks with water, unsweetened tea, or fresh fruit juice is one of the single most impactful dietary changes you can make.
6. Get regular screening. If you are over 40, overweight, or have a family history of diabetes, get your blood sugar checked at least once a year. Early detection of pre-diabetes allows for lifestyle interventions that can prevent or delay progression to full diabetes.

And government can implement policies — such as sugar taxes and mandatory food labeling — that make the healthy choice the easy choice.
Diabetes is a serious condition, but it is not inevitable. With knowledge, awareness, and consistent action, individuals and communities can significantly reduce its impact on Pakistan's health and economy.
